Our medical practice operates as a fee for service cash based practice. In Arizona, Naturopathic physicians may not be considered a Provider on most Health Insurance Plans. However, a majority of insurance companies do consider NMDs as out–of–network providers. At the time of service, you pay for services rendered and Dr. Stevens provides you with a copy of your service summary which includes the appropriate medical codes (ICD-9, CPT) required for reimbursement. You may then submit this information to your insurance company to request reimbursement for your Naturopathic Healthcare services. Payment is required at the time of service. If you are unable to pay in full, you may inquire about a payment options. Payments may be made with cash, check or credit card.
